Electronic Filing
The Illinois Department of Insurance works to ensure that all insurance companies, HMOs, producers selling insurance in Illinois and other regulated entities obey state insurance laws. The Department provides consumer information and investigates complaints about companies and producers. Requests can be filed electronically via the Message Center where consumers can also check the status of requests and update information.
ATTENTION: A request for assistance may be filed by the covered individual (or parent/guardian for children under 18) or their authorized representative if specific written permission is provided. Any person who files a complaint or grievance under false pretenses may be subject to criminal or civil action as the law may allow.

When your completed form is received, the Department will initiate an investigation, assign a file number and notify you. Please use this number on all future correspondence with this Department.
Please allow four to six weeks for completion of our investigation.
