Electronic Filing

The Illinois Department of Insurance works to ensure that all insurance companies, HMOs, producers selling insurance in Illinois and other regulated entities obey state insurance laws. The Department provides consumer information and investigates complaints about companies and producers. Requests can be filed electronically via the Message Center where consumers can also check the status of requests and update information.

ATTENTION: A request for assistance may be filed by the covered individual (or parent/guardian for children under 18) or their authorized representative if specific written permission is provided. Any person who files a complaint or grievance under false pretenses may be subject to criminal or civil action as the law may allow.

Insurance Consumers

An insurance consumer is any person, business or legal entity that is covered by an insurance policy including HMO, health, life, property, business, automobile and other policies.

If this is your first time filing an insurance assistance request electronically, you must create a Consumer Message Center account.

Health Care Providers

If you are a health care provider, please use Message Center to file complaints related to your patient's claims. Use the button below to create a special "Health Care Provider" account.

Note: While the Department encourages provider complaints as an avenue to assist health care providers and consumers and as a method to track market conduct of insurers and HMOs, complaints submitted by collection agencies who have bought the claims are not accepted.

Other Service Providers

A Service Provider is a business that is compensated directly or indirectly through insurance claims for services (other than medical) provided to parties covered by insurance policies.


Service Providers can file complaints related to insurance claims on behalf of their clients. Written authorization from the insured party must be provided.


Note: While the Department encourages provider complaints as an avenue to assist service providers and consumers and as a method to track the market conduct of insurers, complaints submitted by collection agencies who have bought the claims are not accepted.

When your completed form is received, the Department will initiate an investigation, assign a file number and notify you. Please use this number on all future correspondence with this Department.

Please allow four to six weeks for completion of our investigation.
